Agreement to Terms

By scheduling an appointment, receiving services, or using the website of Platinum Aesthetics ("we," "us," or "our"), located at 1809 W State St, Ste B2, Room 103, Pleasant Grove, UT 84062, you agree to the following Terms of Service. Please read them carefully.

Services

Platinum Aesthetics provides aesthetic treatments including but not limited to injectables (Botox, Dysport, dermal fillers, Sculptra), SkinPen microneedling, facials, Hydrafacials, chemical peels, and skin consultations. All treatments are performed by licensed professionals. Results may vary by individual.

Appointments & Scheduling

All services are provided by appointment only. You may book appointments by calling or texting 385.269.2988 or through our online booking system.

Cancellation Policy

We require a minimum of 48 hours' notice for any appointment cancellations or rescheduling. A $50 cancellation fee will be charged for no-shows or appointments cancelled or rescheduled with less than 48 hours' notice. This fee helps cover the time reserved for your appointment and allows us to manage our schedule effectively.

Informed Consent

Prior to receiving treatment, you will be asked to complete intake forms and provide informed consent. You are responsible for disclosing your full medical history, current medications, allergies, and any relevant health conditions. Failure to provide accurate information may affect the safety and outcomes of your treatment.

Treatment Risks & Results

All aesthetic treatments carry potential risks, side effects, and complications, which will be discussed with you prior to treatment. Results vary based on individual factors and are not guaranteed. Platinum Aesthetics is not liable for results that differ from expectations when treatments are performed according to standard protocols.

Payment

Payment is due at the time of service unless otherwise arranged. We accept major credit cards and other payment methods as posted. Membership fees and treatment packages are non-refundable unless otherwise stated in the membership agreement.

Memberships

Membership benefits, pricing, and terms are detailed on our Memberships page. Membership wallet funds do not expire. Membership terms may be updated with reasonable notice to members.

Photographs

Before and after photographs may be taken for medical records. Use of photographs for marketing, social media, or our gallery requires your separate written consent, which you may withdraw at any time.

Limitation of Liability

To the fullest extent permitted by law, Platinum Aesthetics shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services or website. Our total liability shall not exceed the amount paid for the specific service giving rise to the claim.

The content on our website is for informational purposes only and does not constitute medical advice. Information about treatments, pricing, and availability is subject to change without notice.

Governing Law

These Terms of Service are governed by the laws of the State of Utah. Any disputes shall be resolved in the courts of Utah County, Utah.
